By using the App, you agree to the terms of this Privacy Policy.

• Phone number
• Organization or employer (if applicable)
• Login credentials

Referral & Care Information

If submitting a referral, users may provide limited information related to patient care coordination. Users are responsible for submitting only authorized and appropriate information.

Device Information

• IP address
• Device type
• Operating system
• App usage analytics

We do not collect financial payment information through the App.

2. How We Use Information

We use collected information to:

• Process and coordinate home care referrals
• Communicate with caregivers and referral partners
• Improve app functionality
• Provide customer support
• Maintain system security
• Comply with legal and regulatory requirements

We do not sell personal information.

3. HIPAA & Health Information

Peachtree Care Health Services is a Licensed Georgia Medicaid Home Care Provider.

The App is intended to support referral coordination and communication. Users should only submit information they are authorized to share.

Where applicable, Protected Health Information (PHI) is handled in accordance with applicable federal and state regulations.

4. Data Sharing

We may share information with:

• Authorized PCHS staff
• Care coordination partners (as required for services)
• Technology vendors supporting app operations
• Government agencies if required by law

Important SMS Policy: No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

5. Data Security

We implement administrative, technical, and physical safeguards designed to protect user information.

However, no system can guarantee absolute security.

6. Data Retention

We retain information as long as necessary to:

• Provide services
• Comply with legal obligations
• Resolve disputes
• Enforce agreements

Users may request account deletion at any time (see Section 8).

7. Children’s Privacy

The App is not intended for use by individuals under 18 years of age.
