If you wish to delete your PCHS CareConnect account, please follow the steps below.
How to Request Deletion
1. Email: info@peachtreecarecenter.com
2. Subject line: “App Account Deletion Request”
3. Include:
- Full Name
- Email associated with your account
- Phone number (optional for verification)
What Happens Next
- We will verify your identity
- Your account will be permanently deleted within 7–14 business days
- Associated login credentials will be disabled
